The <i>sudden pains surrounding my left flank, for months October onwards.\u00a0<\/i><\/p>\n<div id=\"attachment_11064\" style=\"width: 1290px\" class=\"wp-caption alignnone\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-11064\" class=\"wp-image-11064 size-full\" src=\"http:\/\/nutritional-humility.me\/wp-content\/uploads\/2024\/11\/Carb_allowance-feat-meal-pasta.webp\" alt=\"What started it all. Never a problem before.\" width=\"1280\" height=\"800\" \/><p id=\"caption-attachment-11064\" class=\"wp-caption-text\">Speculation: what started it all. Never a problem before. But suddenly problematic.<\/p><\/div>\n<h2 class=\"p1\"><b>October - December 2024 (potentially onwards).<\/b><\/h2>\n<p><i>It all began when I finished an <strong>Extended fasting 40+ hours<\/strong> mid October. Feature meal as PWO (post workout) and breaking the fast ~ pasta meal with tomato paste. Something I haven't had for quite some time. <\/i><\/p>\n<p><i>All of a sudden things slowly turn south. <\/i>A day or two after I felt radiating, nagging pains, notably on left side of <em><strong>middle<\/strong><\/em> quadrant.<\/p>\n<p class=\"p3\">First week was mild. But fast forward next two month(s) - quite debilitating that even made walking short distances difficult without short burst of spasms or jolts of pain resembling a disc slip (post trauma \/ training injury).\u00a0<strong>But here's the strange thing ~ it came out of nowhere. There was no \"injury\" nor impact trauma per-se.\u00a0<\/strong><\/p>\n<p>Yet a day or two after that feature meal, the pains emerged. Out of the blue.<\/p>\n<blockquote>\n<p class=\"p3\">Given my<strong> long term condition (AS) ~ I am very well aware of the overall pain distinction. <\/strong>Much less (thankfully) of a \"freezing\" jolt of shock through the entire spine. However, even though I \\remain <strong>tense, almost constantly on a daily basis<\/strong>\u00a0to anticipate its pain whenever I <strong>laterally-flex (eg. bending at the hips laterally, pressing on the obliques)<\/strong> .<\/p>\n<\/blockquote>\n<p class=\"p3\">What I did was the following, from most to least obvious. Immediately upon the first few days of notice:<\/p>\n<ol class=\"ol1\">\n<li class=\"li3\"><em>Continue Eliminating all oxalate foods. (Which I already reduce as an overall intake in recent \/ prior years anyway).<\/em><\/li>\n<li class=\"li3\"><em>Complete elimination of Vitamin D.<\/em><\/li>\n<li class=\"li3\"><em>High emphasis on calcium intake. Mostly from cheese and yoghurts.<\/em><\/li>\n<li class=\"li3\"><em>High(er) water intake. 4 to 5+ litres. Per day.<\/em><\/li>\n<li class=\"li3\"><em>High magnesium dosages - combining both the oxides and chelated, with Vitamin B6.<\/em><\/li>\n<li><em>HIGH Magnesium citrate and potassium\u00a0 citrate (powder).\u00a0<\/em><\/li>\n<li class=\"li3\"><em>(On later weeks) ~ higher potassium (chloride) supplementation.<\/em><\/li>\n<\/ol>\n<p class=\"p3\">Within as little as five days I felt relief and even training sessions went unaffected. However came next week, even while maintaining all the above ~ the <strong>pain returned. And things got progressively worse.<\/strong><\/p>\n<blockquote>\n<p class=\"p3\">Sleeping became painful as even a minor torso rotation, back and forth ~ triggered the same <em>burst <\/em>jolt of pain on the flank. This soon became a daily thing.<\/p>\n<\/blockquote>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-11072\" src=\"http:\/\/nutritional-humility.me\/wp-content\/uploads\/2024\/11\/November_ultrasound_kidney.webp\" alt=\"\" width=\"1302\" height=\"824\" \/><\/p>\n<h2 class=\"p1\"><span class=\"s1\">My imaging results and what the \u201cconventionals\u201d say<\/span><\/h2>\n<p class=\"p1\"><span class=\"s2\">I managed to secure both Ultrasound, CT (and later on - MRI) scans about one and quarter month of this pain. And what \u201cthey\u201d, <a href=\"http:\/\/updoc.com.au\">the contracted GPs of Updoc\u00ae\u00a0<\/a> had to say left me feeling mixed. <\/span><\/p>\n<p class=\"p1\"><span class=\"s2\">Ultrasound did not come up with anything unusual.<span class=\"Apple-converted-space\">\u00a0 <\/span>Likewise with the CT scan.\u00a0<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">On one hand it is half comforting to hear that my case is not that of typical renal stones. Apparently, the pain should be so overwhelming ~ I\u2019d be immobilized. <strong>I suspect it'd be to the level of\u00a0 Dr Eric Berg\u2019s experience - <a href=\"https:\/\/www.youtube.com\/watch?v=gx375QamTLE\">rounds of tylenols, and hours of lying flat on the ground.<\/a><\/strong><\/span><\/p>\n<p>There are however interesting cues for me to look up and learn. Though be forewarned ~<strong> these are just for my own speculative insight.\u00a0<\/strong><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-11125\" src=\"http:\/\/nutritional-humility.me\/wp-content\/uploads\/2024\/12\/CT_Scan.webp\" alt=\"\" width=\"1280\" height=\"801\" \/><\/p>\n<ul>\n<li><em>I don\u2019t feel anything unusual on my right side but here ~ <strong>\u201cRiedel\u2019s Lobe\u201d<\/strong> indicates some sort of rare liver \u201c<strong>anatomical variation<\/strong>\u201d, that predisposes me to bloating.<\/em><\/li>\n<li><em>Thankfully, I have not had any serious <strong>sciatica-<\/strong>like episodes. But <strong>\u201cPhlebolyths\"<\/strong> indicates there is some vascular calcification in the left pelvic regions. I\u2019m not sure if this correlates to where my pain is, which is <strong>defenitely located<\/strong> <strong>higher up in the flank, <\/strong>somewhere between the lower ribs.<\/em><\/li>\n<li><em>A little trivia or perhaps confusion here is medical terminology. \u201cPaucity\u201d to me I thought heavily implies that something is \u201cthere\u201d. In this case the \u201cintra-abdominal fat\u201d blocking accurate diagnosis. Surprisingly, upon more reading it actually means the opposite. \u00a0I would never condone myself to become \u201cfat\u201d just for the sake of \u201cbetter diagnosis.<\/em><\/li>\n<\/ul>\n<h2><strong>And then...MRI.<\/strong><\/h2>\n<p>Arguably one of the worst \"spending\" one ever has to spend during Christmas (next to car breakdowns, or emergency plumbing repairs) is <em><strong>more<\/strong><\/em> unwanted health expense.<\/p>\n<p>In my case, by about second month through (being patient with the flank pain)<strong> I've already wasted<\/strong> <strong>$150 just <\/strong>jumping through hoops in the Updoc\u00ae telehealth platform. Just trying to understanding <span style=\"text-decoration: underline;\"><em><strong>what<\/strong><\/em><\/span> this flank pain is. The very first and arguably most important step about learning is to <span style=\"text-decoration: underline;\"><em><strong>identify. <\/strong><\/em><\/span><strong><em>EVEN IF WE CANNOT explain it, at the very least, identifying the \"what\" ~ what type or kind (of pain) is key.<\/em><\/strong><\/p>\n<blockquote><p>Mine, to this day is still what many would call <strong>idiopathic. <\/strong>Meaning ~ we don't know. At least not yet.<\/p><\/blockquote>\n<p>So there is one more thing left in my list.<strong> My<\/strong> <strong>lumbar spine. <\/strong><strong>Considering it was over ten (well technically eleven) years ago since my last MRI then diagnosis on AS, in 2014.\u00a0<\/strong><\/p>\n<blockquote><p>So how did it go after an extra <strong>$350 lavish spending? <\/strong>The (next available contracted GP on Updoc) did the usual assurance two weeks later ~ <strong><em>\"Everything's normal\".\u00a0<\/em><\/strong><\/p><\/blockquote>\n<p>I let you all decide and decipher the results below.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-11138\" src=\"http:\/\/nutritional-humility.me\/wp-content\/uploads\/2024\/12\/MRI.webp\" alt=\"\" width=\"1280\" height=\"801\" \/><\/p>\n<p>The only next step, according to their opinion ~ was to simply consider <strong>physiotherapy<\/strong>.<\/p>\n<p>I clearly stated to them (\"everyone\"), Doctor(s) x,y,z ~ that this WAS NOT an injury nor muscle \"strain\". If it were, I'would have known <strong>exactly<\/strong> what, where and how it manifests. But my case - it <strong>came out of nowhere.\u00a0<\/strong><\/p>\n<p class=\"p1\"><span class=\"s2\">The spasm and mini jolt(s) of pain meanwhile, remains frequent enough I can't ignore it. Even sneezing - is something I had to be very conscious. Likewise even <strong>going to sleep\u00a0<\/strong>~ brought me some anxiety. <\/span><\/p>\n<blockquote>\n<p class=\"p1\"><span class=\"s2\"><strong>These level of tension <\/strong>never occcured before in my life. Sure, I've gone through L4\/5\/S1 disc injuries in the past. But confide in comfort knowing and identified <em><strong>what led<\/strong> to it.\u00a0<\/em><\/span><\/p>\n<\/blockquote>\n<p>This on the other hand, is something else altogether. So many occasions I'm convinced it's nerve impeachment. After all <em><strong>something is pressured somewhere in there.<\/strong><\/em> Other times, I still think it's oxalate related. Reintroducing peanut butters and sadly, <strong>even gelatin powders <\/strong>for instance, seem to trigger these overall tension.<\/p>\n<p>However few optimism left for me at the very least, I only have questions as the only trail.<\/p>\n<h2 class=\"p1\"><b>\"Mast cell activation\"? \"Oxalate Dumping\"?<\/b><\/h2>\n<p class=\"p1\">Recalling basic theory - the body does generate its own oxalate through <b>Glyoxalate<\/b> pathway. As protein utilisation and oxidation proceeds, particularly from this specific amino acid hydroxypoline ~ it becomes prone to glycation, <strong>if it also happens to<\/strong> interact with another (suspect \/ culprit) ~ <strong>Ascorbic Acid or simply Vitamin C<\/strong>. <b>Glyoxalate <\/b>is then produced as the end product, then becomes direct precursor to the oxalate crystalization (upon which as it implies) ~ oxalate \"<strong>crystalizes\" <\/strong>by\u00a0urinary supersaturation (fancy word for <em>dehydration<\/em>).<\/p>\n<p class=\"p1\">There is however mystery in all this. Enter \"dumping\". This is where the symptoms typically associated with <b>toxicity, immediately becomes apparent soon after 100% elimination of dietary oxalates. <\/b>Back pain, depression \/ brain fogs and overall general \/ unexpected malaise.<\/p>\n<p>Understanding how or why the \u201cdumping\u201d happens, is still a confusing part in all of this.<\/p>\n<blockquote>\n<p class=\"p1\"><i>There are those who believe the dumping symptoms appear greater in those <\/i><a href=\"https:\/\/open.spotify.com\/episode\/4hsquSc5uLB1LLrsLPzsku?si=391e8173ec974bd2\"><span class=\"s1\"><i>who exclude all oxalate intakes too fast, versus those who gradually slows them.<\/i><\/span><\/a><i> As to how and why this occur is not known (at least to me).\u00a0<\/i><\/p>\n<\/blockquote>\n<p class=\"p1\">Retrospecting my prior years (on\/off dietary oxalate intakes in short periods of time) ~ I can definitely <strong>re-create classic symptoms typically associated with<\/strong> <strong>oxalate-sensitivity<\/strong>. Occasional lower back (nagging) pains ~ that usually resolves within a day or two of dietary exclusions.<\/p>\n<p class=\"p1\">Then there is another thing in my list of suspect(s). <strong>\"Mast cell activation syndrome\"<\/strong>. My early understanding suggests it is something to do with over accumulation of white blood cells that have tendencies to \u201c<strong>get stuck<\/strong>\u201d longer than it has to within actual tissues ~ both organs and connective. Excessive inflammatory responses then potentiates, which also reportedly contribute towards oxalate synthesis. I suspect that my ongoing AS may <a href=\"https:\/\/www.google.com\/search?q=mast+cell+ankylosing+spondylitis&amp;sca_esv=825ec49c2d0202bf&amp;rlz=1C5CHFA_enAU1015AU1015&amp;sxsrf=ADLYWILJr39Bh4hHGL5K-XyeYiHs2mwKng%3A1731730542606&amp;ei=bhw4Z9PcJNuNseMPh6yyuA4&amp;oq=mast+cell+ankyl&amp;gs_lp=Egxnd3Mtd2l6LXNlcnAiD21hc3QgY2VsbCBhbmt5bCoCCAAyBBAjGCcyBhAAGBYYHjILEAAYgAQYhgMYigUyCxAAGIAEGIYDGIoFMgsQABiABBiGAxiKBTILEAAYgAQYhgMYigUyCBAAGIAEGKIEMggQABiABBiiBDIIEAAYgAQYogQyCBAAGIAEGKIESJMTUKEHWMUMcAF4AZABAJgBiAKgAZUJqgEFMC4xLjS4AQPIAQD4AQGYAgagAroJwgIHECMYsAMYJ8ICChAAGLADGNYEGEfCAgsQABiABBiRAhiKBcICERAAGIAEGJECGLEDGIMBGIoFwgIFEAAYgATCAgoQLhiABBgUGIcCwgIIEAAYFhgKGB6YAwCIBgGQBgOSBwUxLjAuNaAH4ys&amp;sclient=gws-wiz-serp\"><span class=\"s1\">have some associations with it. <\/span><\/a>But so far it's quite nuanced and heavy reading for me at the moment.<\/p>\n<h1 class=\"p1\">Where \/ How I am now<\/h1>\n<p class=\"p1\">(Updated 29th December)<\/p>\n<p>Not all, thankfully, is \"doom and gloom\". Towards end of December, I find there have been few variable(s) I recently find that, despite how unintuitive some of these were ~ nonetheless <strong>helped contribut some way shape or form at relieving the flank pain tensions.\u00a0<\/strong><\/p>\n<p><em>Although these, I shall write with a large asterisk, with degrees of uncertainties. Your milleage of course remains yours. I do not know that simply adhering to these continuously will fully absolve the problem. Hence just a disclaimer much of this is still mystery to me.\u00a0<\/em><\/p>\n<h3><strong>Vitamin A..... But only from carrots.<\/strong><\/h3>\n<p>...Not Livers. For many years Livers have always been my go to source as \"multi-vitamin\", in whole food form. <strong>Given also it is widely and well-though, that Retinoic Acid,<\/strong> as opposed to precursors such as caretonoids \/ beta carotenes, are superiro and more bioavailable form of Vitamin A<strong>. <\/strong><\/p>\n<p>However in just space of two weeks I tried mega dosing Vitamin A; drawing comparisons from either <strong>livers (chicken liver pate DIY as usual) <\/strong>or <strong>carrots<\/strong> (as dessert and feature meals).<\/p>\n<blockquote><p><strong>Recalling my journalling\u00a0 - Carrots<\/strong> but NOT chicken livers ~ somehow brought me more ease and pain relief next day. I can confide \/ attribute to these simply through comparing through my notes among days where I consumed <strong>ONLY AND EXCLUSIVELY ~ carrots VS chicken livers.\u00a0<\/strong><\/p><\/blockquote>\n<p>I know and came to accept this, for me at least \u00a0~ having experimented and compared with days that I only eat ONE OR THE OTHER.<\/p>\n<h3><strong>Premethylated Multi Vitamins.<\/strong><\/h3>\n<p>In general I have been somewhat hesitant of taking multi vitamins (MVs). UNLESS, as per my last year's write up - that if it is specially formulated with <strong>premethylated components<\/strong>, then I may be compelled to stock up one or two. This year thanks to iherb's occasional heavily discounted promotions I was able to secure discounted trials of these premethylated vitamins.<\/p>\n<blockquote><p><strong>Once again <\/strong>in retrospect to my own self-journalling - comparing between days where I took these MVs versus those I did not ~ <b>clearly there were improvements in overall<\/b> mobility.<\/p><\/blockquote>\n<p>I speculate there might be the pre-methylated versions of certain B vitamins at play here. Which, as I am aware from my nutrigenomics recommendations\u00a0 to take certain pre-methylated supplementations ~ B6 as P5P, methyl folate (B9), methylcobalamins (B12), just to name a few. Perhaps there is some sort of nerve health at a compromise here? Certainly more reading is warranted for me here.<\/p>\n<h3><strong>Magnesium? Bad news for me. <\/strong><\/h3>\n<p>Saving the surprise for last and counterintuitive to 99% of people, including experts.<\/p>\n<p>I shall caution that this is still early days \/ speculation for me.<\/p>\n<p>What I have gone through throughout first month (since the pain started) was copiously taking in Magnesium Oxide (the \"cheap\" over the counters, with cofactors B6).<\/p>\n<p>Back then - I thought the magnesium oxide was all that I needed. Confusingly ~ things didn't turn out\u00a0 the way I anticipated. Neither improvements nor relieff on the overall flank pains (and\/or the \"tension\").<\/p>\n<p>Once I've ran out of Magnesium Oxide supplement. I thought to myself (okay), let's try going through several weeks WITHOUT it. And that was when I found out the above first two (2) helpers. that 1). more carrots and\u00a0 that 2). the mutivitamins somewhat did contribute to pain relief.<\/p>\n<p>So I went back to my journalling again and see what else <em><strong>I could find a pattern.\u00a0<\/strong><\/em><\/p>\n<p><strong>And what I found ~ was magnesium.<\/strong> If I knew something odd happened as soon as I introduced a variable, then I would've written it down. And magnesium, appears to be a strange suspect here.<\/p>\n<blockquote><p>Specifically <strong>magnesium chelated. Despite what everyone thought it is the better version<\/strong>- this appeared to <strong>triggered more tension and stiffness. Thus - more flank pain<\/strong> episodal burst(s) and spasms. As to how or why this is happening is beyond me.<\/p><\/blockquote>\n<hr \/>\n<h2>But other than that... What are the things that I still\u00a0<strong>cannot do?<\/strong><\/h2>\n<p class=\"p1\">Sleeping do get disrupted from time to time. <strong>Some anxiety still lingers where I had to pay some attention on how I should position my sleep.\u00a0<\/strong><\/p>\n<p class=\"p1\">On matters of training (more in next part), I have to be mindful on <strong>pull-down<\/strong> exercises that involves compressing the laterals and obliques. Lat pull downs and assisted pull ups for example, are no go. <strong>I would say lateral flexions (eg. side bends \/ lateral hip bends) would not be safe for me.\u00a0<\/strong><\/p>\n<p>Let's wrap things up in the next part.<\/p>\n<p>Live-It-Forward,<\/p>\n<p>AW.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Continued off from first part of 2024 annual food for thoughts -
